NOTE: Survey results will be computed for the following geographic areas. If you have employees in more than one area, and your average practices do not vary significantly by area, you may indicate your number of employees in each employee group for each area under "Employees by Area" before responding to the remaining questions. However, if your average practices vary significantly for each area, please complete separate survey input and employee counts for each area.

Area Definitions:

Area 1 - Portland Metro Area
Clackamas, Clark, Columbia, Cowlitz, Multnomah or Washington Counties

Area 2 - Mid-Willamette Area
Benton, Lane (Non Coastal), Linn, Marion, Polk or Yamhill Counties

Area 3 - Surrounding Area
Baker, Clatsop, Coos, Crook, Curry, Deschutes, Douglas, Gilliam, Grant, Harney, Hood River, Jackson, Jefferson, Josephine, Klamath, Lake, Lane (Coastal), Lincoln, Malheur, Morrow, Sherman, Tillamook, Umatilla, Union, Wallowa, Wasco or Wheeler Counties
